Mohnish Gupta, an HIV-AIDS physician, left a government hospital position for a pivotal position at The Global Health Engine (GHE), an international organization renowned for coordinating and promoting scientifically based medical efforts on a global scale. GHE provided Gupta with career prestige and a comprehensive package that doubled his government salary, but the job was exhausting Gupta, jeopardizing his health, and robbing him of precious time with his family. Just as Gupta decided to resign from GHE and return to the government hospital, GHE offered Gupta a promotion that came with a significant salary increase, more responsibility, and even greater professional prestige. The promotion was attractive: an opportunity for Gupta to have yet more influence in his field of practice and an income that would support Gupta’s plans to provide overseas education for his children. But the promotion also came with significant costs: a lack of autonomy in his work life and significant interference with what little remained of his home life. Should Gupta accept the promotion or continue with his plan to resign from GHE and return to greater professional autonomy and a more balanced work life but lower income at the government hospital?
